I cannot add a broadband connection that requires a username and password
because the option is GRAYED OUT.

How can this be fixed?

This is the usual reason:

You Cannot Create a Network Connection After You Restore Windows XP

The usual fix is to re-apply your current service pack ( SP1 or 2 )

But also check this:
